Always louder: continue getting louder.
Printed below or above the staff at the start of a long build, sometimes with repeated cresc. markings or a series of rising dynamics. It holds until a target dynamic or a new instruction is reached.
Louder and louder: a sustained crescendo over a long span, often across repetitions of a figure. It tells the performer to keep growing even when individual phrases seem to end. State the goal (bis ff), and pace the growth through orchestration as well — adding instruments makes immer lauter believable without forcing tone.
